Be careful when dealing with this company. I turned over my insurance information and was told I would only be out of pocket by the amount of my co-pays. A year later I get a $400 bill, call to discuss, they tell me they will investigate, and then turn me over to a collection agency. Do not trust what they tell you, get everything in writing.
